Need all compassion club leaders to assist us by filling out contact sheet

Hello Family,

Could you please complete this form and return it back to me. The MMMA Compassion Club Committee will use this information to improve and increase information from the administrative office. We would like to create a database that complete with information of active clubs. If you have any questions, please feel free to contact me. The form is available under attachment.

Hi Brad was at the CC meeting for waterford. Steve mentioned they needed someone to run it. Don't know if anyone ever did but would be willing to.

brad has nothing to do with that.
You want to get a hold of someone of the Macomb Oakland Compassion Club - like Steve, jack, or Will. Check out the Roll n Bowl (next event) otherwise next MOCC meeting is around January.

They do want someone to run it, you might already have some support from local shops in the waterfold area.

Update: I put up a electronic version. Not very pretty but it would work. Just fill it out save it and email it back to Moe.

It really should be a electronic, editable form. So we can fill it out and just email it back.
Lots of people don't have printers anymore.

You have never sent this form to me, I actually just created it last week. Be a pal and take ten minutes and fill out, forgive I might have been medicating and deleted the email (haha). All jokes aside, We are trying to get a fresh start and compile complete information for clubs. Did you review the form, let me know what you think.

I'm a computer illiterate, how about an email to send this thing back. I can't open one attachment, I can't seem to move Todd's form to the mmma site to use the contact to send it. Send me an Email I can send this to please. Thanks for the help Todd.

Gotta make these projects more user friendly, Todd's right.
