They ask for recent medical documentation of your illness ( past 3 years), and a picture I.D.
It is $150.00 for the appointment. if you don't get approved you don't pay.
are next appointment day is Friday May,15th, call and come in the same day.
if you have your records you can see us tomorrow!
If you need medical documentation we do that too!

Amyone use this Clinic?
Like I mentioned in my older post,I am going to this Clinic next week. Anybody have an experience with them? Have I picked the right place? Thank you.

i've been in contact with these guys for about 2 weeks now, and by these guys i mean corey, haha.
i also will be going wednesday to get squared away.
i can say that i told my brother about them last week and he was working in and out of detroit, so after his primary doc denied his MM request he faxed over is medical records to them and they saw him friday.
he said it was extremely easy, and they were all really nice. they are looking to help you so expect to be helped if you've got an appt.
you definitely chose the right place.

I got my appt for next Wedsnesday. I think I will be approved. No more fear that I could be arrested is unbelievable and takes a great burden off me.I will no longer be a criminal.And hell yes, we head towards death the day we are born.
