Just moved to MI. How do I get my patient card?

I only have a po box is this ok?

 

Driver license changed to Mi license.

find a doc to certify me. WHat ailments are also normally allowed? I see the main ones on App. I have blood pressure issues, apnea, overweight,  vertigo.

 

then apply?

You need to have a chronic condition that you see a doctor for regularly. Something that causes a lot of pain is what most qualify with.

Post Traumatic Stress Disorder was also added to the qualifying conditions list, but it is not listed in the law or on the application form.
