Hello Everyone!

Dr. Bob will be holding a clinic at the Days Inn in Clare Michigan on Tuesday, June 2, 2010 from 4:00 - 10:00 pm.

The cost is $200 for initial recommendations and $150 for recertification (renewals).

You must bring your medical records with you in order to get a recommendation.

Dr. Bob would like an idea of how many people plan to attend the clinic. He says he is able to get through 30 people in 4 hours. Please reply to this message so I can give him an idea of how many people to expect so he can plan accordingly. Also, please indicate what time you plan to attend.
