Petitions To Add Conditions

Departmental Authority to expand conditions covered:

New conditions can be specified in the future. Rule 333.131 requires the Michigan Department of Community Health (MDCH) to have a procedure for accepting petitions from the public to include additional medical conditions and treatments. The procedure must include public notice of hearings on each petition, opportunities for public input, and a reasonable timeline (180 days) for approval or denial.

 

Is there a petition form available ?

 

Also I cant find Links to other forms other than just going to MDCH

 

There used to be a links to a petiton form to add a condtion, change forms, renewals, application form, ect...

 

Unless Ive missed it maybe it wasnt added to this site...

 

Anyway, I think we should provide a clear path for people with conditions not currently accepted...link to petition form, instructions on what to do, ect

 

THe other links would be great too...unless they are already here...but maybe they could be more obvious or specific

In another thread someone said to just write a one page letter to the MDCH, explain the condition and how MM helps, and ask them to add it. There doesn't seem to be an actual petition as far as I can see.

is there a committee in the mmma, or chairperson or something, that meets to discuss additions? or is it just randomly up to patients, as individuals?? and then what happens? does it go before a medical review board? anyone have any success getting conditions added?

It would make it easy for people to use We need to give people a path to follow

 

MDCH forms link would also help make it easier to change adress, caregivers, apply for cards, and renew cards
