Change Of Cg Form

Hi everyone.

 

I currently have a caregiver and would like to make myself my caregiver. I was curious as to which options to select on the form. In sec. B of the form, do i select "remove caregiver" and then in sec C select "change to patient"?

 

Or, in section B do i select "add/change caregiver" and in section C "change to new caregiver"?

 

Or, am i nuts and just dont know what im talking about, lol?

remove caregiver change to patient seems like the best choice.

check the "change to patient" box and write "PATIENT" where caregiver name would go. then there's no confusion
