I have a patient who needs to appoint a caregiver and process seems to be unclear. How to apply? how long untill its legal?

You have a Patient ..... who needs to appoint a caregiver ?

 

then they are not your Patient Correct ?

 

A Patient will appoint a person as their Caregiver

 

and that person has to fill out the " Caregivers Attestation "
