strangluv Posted August 3, 2011 Report Share Posted August 3, 2011 When sending in a Caregiver Attestation does the address need to match the address on the Driver's License? Can a PO Box be used? While the state cannot seem to finger out a way to put my photo on my card, they have assumed that all my patients, their friends and any dispensaries they might visit deserve to see my address. Not to mention the likelihood of some patient losing that darn near indestructible plastic card. Join the MOCC, great group and patient's come first as a member it has come to my attention that the clubs p.o.box is available for this pourpose can you imagine 5,000 caregivers using that address?!?! also, I like MY patient card to have the same address as my licence, so I use that address for myself ONLY, p.o.box for caregiver cards Quote Link to comment Share on other sites More sharing options...
