No Reply From Patient

I have a patient that I haven't heard from in months, I have had no reply to numerous calls and texts, I stopped by her house and was told by a neighbor that she had moved. I am changing addresses and have all the paperwork in order from my other patients except hers. Does anyone know the procedure for a caregiver to release a patient? I can't seem to find any info or a form that addresses this issue. Thanks for your response.
