? Regarding State Fees

a friend of mine told me a friend of thiers was able to qualify for the $ 25 state fees regarding disabilties, this firend claims that thier friend qualified for the ? 25 dollar renewal fee for having a bridge card , sounds fishy but who knows to make this long story short they said that if you can show u r recieving any type of state assistance that it automatically qualifies you. would appreciate any info on the subject, thanx

To get the reduced fee a bridge card will not do it.

 

$100.00 APPLICATION FEE or $25.00 APPLICATION FEE if patient is currently enrolled in Medicaid or receiving SSI or SSD, and submits the appropriate supporting documents

Check or money order only. Make payable to “State of Michigan—MMMP.” Do not send cash.

 COPY OF DOCUMENTATION VERIFYING RECEIPT OF BENEFITS, IF SUBMITTING $25.00 FEE

Acceptable: Current Social Security Administration document stating the patient receives disability benefits, MI Health card or other Medicaid health plan card (FULL Medicaid Only)

NOT ACCEPTABLE: Medicare card, Bridge card, Bank statements, Social Security IRS Form 1099, Social Security yearly benefits statement, VA disability, Retirement benefits

Not a Bridge card, but a Michigan Health Card, will get you the discount. You have to send a copy of the card with your application. You can also qualify with certain paperwork from Social Security if you're on disability.

 

Just be sure to send in the "letter of determination of disability" not the statement of benefits amount. If the letter doe not say you are on disability you will get a denial letter asking for the dertmination letter. and you 21 days starts over from the "new" application you have to resend.

thanx everyone, i thought what my friend was describing seemed off, he is planning to try it with his bridge card proof, i myself am goin through the process of applying for ssd as my MS has been progressing, so once the long dragged out fight is over than i should be able to qualify myself, good luck to all be well and stay well medicated . :bong7bp:

Good luck and God Bless you, hopefully your decision is not a long one. I was very lucky when I applied in 96, MS, I was approved for SSD after visiting their state doc for an exam, here's to hoping yours goes as quickly.

I think there is some sort of confusing naming of forms by the State since this is a copy from them I see now used to describe what is acceptable or not . As usual they make things hard for the patient why not just say exactly what form they want . I have sent in my "Statement of Benefits " every year it says on the letter this can be used for proof needed to qualify for programs based on need or something like that . I haven't had a problem nor received a denial ever , it says statement of benefit . My letter of determination as total and permenently disabled would be 10 years old . They recently cashed my $25 check over 21 days ago for the 4th year . I am told they arent' sending out those approval letters they announced a few months back to be as good as cards - anyone know ? Hopefully next year my card will be good for 2 years and I prefer using a cannabis specialist so one person doesn't control all my medications especially those who have little understanding of the laws and familiarity with the treatment . The state merely determines through the Doctor whether there is a possibillity of benefit and I self supply and treat as the law allows after some guidance which is so much better then traditional regulated care and dosing arrangements . If there is a dosing problem I can correct it ASAP based on my knowledge of my body and its reaction . You can get your STatement of Benefit anytime online it takes 10 business days to arrive so keep a few on file . They work for lifeline discounts from the phone company too and for credit which is a trap for individuals who can no longer work or require " sheltered employment " .

My insurance is called caresource. Wonder if thats the same?

Adult Medical Program (AMP) provides basic medical care to low income adults who do not qualify for Medicaid. There are asset and income limits. Some counties have a county health plan that the person must be enrolled in to receive AMP benefits. CareSource is an AMP

not begrudging social security recipients their ss disability, not for a minute.

what i have a problem with is that in the eyes of michigan's mmj law disabled vets count for nothing when it comes to the $25 fee vs the $100 fee for disabled vets.

guess i'm not sophisticated enough to understand the discrepancy.

 

any thoughts from vets out there?
