Caregiver Fee On New Form Checklist

This may be a dumb question......

On the LARA checklist page there is a $60 patient fee. Just below that is a Caregiver fee $25. Who pays this $25? I'm about to send my renewal in $60, I do have a caregiver, do I have to pay this $25 each time I renew and have a caregiver? Or is the fee for Caregivers? I'm confused.

Zap, is it for new Caregiver/patient or does it get paid every renewal? Don't remember paying $25 for a Caregiver before.
