So do we check the box to drop current cg 10$fee then check box to change cg 10$ patient fee, 25$ cg fee, then box for plant possesion 10$ fee for a total of 45$? Registry ID number for patient is required on form already. But, do the checklist. p.s. I mean, if you sent scans of everything, it isn't like they will reject it.
