Wondering what exactly is disclosed or filed on the forms for membership at the Ypsi dispensary? Anyone have a copy or do they even allow you one? How much do they charge month/year? Is their a disclaimer or any rights waving? Just trying to understand more of the process. Would love to see a photocopy of the membership forms.

name, address, and the basics. patient/caregiver card number, dates issued/expired, and medical conditions. a section for them to document what strains you are using. dont have a copy, but i don't see why one couldn't get one from them. last i knew, it was $12/year for memberships, but that was back when they opened. maybe someone has a bit more knowledge on this. hope it helps.
