I would like to know if and why you checked the box at the bottom of your MMMP app.

"I additionally autorize the release of my name and date of birth to law enforcement to confirm identity, only if law enforcement has provided the mmmp with my registration number."

 

Also, the MAILING address I put on my paperwork has nothing to do with where I actually grow right?
