Searching For Macomb County Doctor Able To Sign Physican Form

I'm a Macomb county resident looking for a local physican. I suffer from severe chronic ankle pain from sports injuries. Sometimes at night I can barely walk. Sick of taking pills or drinking to ease the pain. I have been recommended a doctor in Mt. Morris that doesnt require a history. If anyone knows a doctor in Macomb that doesnt require a history to sign the needed physican form that would be great. Thanks in advance
