Question About My Cert (Need Answering)

was lookin at my paper work and I just found that the Part of the Cert that states at the top

 

"Physician Certification"

 

My doc completed the 1st half of the Paper .... on the 2nd part "Certification,Signature,& Date"

 

the doc DID NOT sign or date it but on the part below that "Contact person" is filled in by the contact person.

 

 

On the Physician Statement Paper

 

 

Everything is signed with a signature from the doc...

 

 

now To me this seem Odd Should I go back to the place and get this corrected?

nvm i found it the copy i have in my house was not signed but the copy in my car was signed...lol i think it was a copy machine error im recopy the signed one

yes i have everything in packets each packets is identical with all apps,physician certs all signed, copy of license copy of check before being cashed and copy of check cashed and stamped, and a MMMA rules packet on each .
