My Card Never Came! What Do I Do?!

my paitent mailed in all the forms to set me up to grow he got his card renewed 2.5weeks ago but i never received my actual caregivers card does anyone know who i can contact to find out where it is or how to obtain another one?

I would have THE PATIENT contact the MDCH in Lansing. Only the patient can contact the MDCH concerning your card, I don't believe that they will even discuss this with you. Hippa laws are involved here and they are restricted as to who they might discuss the patients needs with...Peace...j.b.
