Card Came With No Care Giver

My card game and on the back it has no caregiver yet it was on the application? Is that standard

If you assigned a caregiver it should appear on your card.

I did and looked at the application as I kept a copy and it clearly showed it in both areas so I guess I will have to check into why my husbands name is not listed as a caregiver

I hear they are making many mistakes. My Patient card was fine, however, my CG card had typos on birth date and address.
