Ok, A friend just asked me about the change of caregiver form. He is using this form Link here He is sending a change of caregiver form to remove his caregiver (section B) I know he is to mark the remove caregiver box but my question is should he fill in all the info for his current caregiver (the one he is removing) or just leave that section blank since he is not assigning a new one? He is just removing the caregiver and changing his plant status to himself. I told him to leave it blank but would like opinions or hopefully someone that has already done this. I also told him to mark the section C to change posession of plants to patient. Thanx for all responses.

yep leave it blank, make sure he marks patient posses plants send in the 10 bucks and good to go,,let the c.g know you sent in change form!
