Need Help With Renewing Card Please

Hello, So I got my card and it expires the beginning of next month. I've read that I'm suppose to get a package in the mail to renew it and haven't gotten anything. What do I do? Thanks.

Follow this link to get the renewal application. Then you will need to see the doctor again and have them sign the recommendation. My link
