Lost Patient Cards

So after my most recent redesign of my room, I forgot where I put my patient's cards. For about thirty minutes I retraced my steps and searched, finally I found them. They had to be there somewhere, but it made me think. As a caregiver with 5 patients, what would I do if I lost my patient's cards (or worse, had them stolen). Would I just write a statement attesting to the loss of the cards to LARA? Would there be a $10 fee for each? Anyone had this happen? Curious because, while I don't expect it to happen, I always like to know answers before the question is asked.

Yes.. you would need to inform Lara of the loss or theft of any cards and replace them for a 10 dollar charge per card lost.. Keep ur hands on them bud.. Peace

A change form would also be necessary for anyone who moves or has a change of address.. so dont forget folks we are to make an address change within two weeks (or so im not absolutely sure on that one) from a move.. :-) Keep within the law
