Is There A Patient/caregiver Agreement

I am curious to know if there is a general patient/caregiver agreement somewhere, it seems like I have read one somewhere but cant seem to find it now. any help would be appreciated. I just think it is best to have everything written down for everyones sake.

Personally if My Wife has to Sign Something

between her and the patient , she does not need

to be their caregiver ...

 

We sit down w/ her patients and we talk ...

we ask what the patients needs are and if we're

able to meet their needs ...

 

We talk about alot of personal things and there

will be a friendship made between them and us ...

 

Jane offers them a FREE Oz of Quality Meds each Month

and if they need another one it's $240.

The Patient is aware that Chit does happen , But knows that we

will do everything in our power to protect them and have their

Meds on a timely manner .

 

We ask that her patients come see Us on a regular basis .

We have them come over for a nice meal and desert .

We feel that Fellowship and Friendship carry's Top Priority ,

along w/ their privacy .

 

Communication is Very Important between Patient and Caregiver .

No ... It's just something a Caregivers or Patients feel

they need one between them ...

 

Personally if our patients needed a contract signed

between us and them , We would NOT have them as a Patient

It seems to me that agreement or not it is good to set the expectations up front. Having it in writing just keeps everyones memory the same.
