Registry Number For Renewal

My patient just contacted me and asked for my caregiver registry number for his renewal paperwork. I don't have my caregiver card for him yet however, I got a caregiver card in the mail today from a different patient /frind I took over for last year for a few weeks. That card has a different registry number than my patient card. Which number would he need for his renewal papers? Thanks

I noticed on the back of my new caregiver card the last set of numbers were the same as my patients first set of numbers. So would my other patients number be cmy first number and his first numbers?

thats correct. I'm fully carded. my patient card and all 5 cg cards have same first number and the patient numbers are the second set

You need the card for the patient that is renewing. I am pretty sure that each card has a different Registry number. My CG's registry number is different than my patient registry number, and that is all on the same card....lol
