What Paperwork To Send To The State

I am just recently adding a new patient. They went to get their certification and brought me the paperwork, but it only has a copy of their drivers license, the physicians certification, the application form, and a check. When i renewed my latest patient, I was required to send in a caregiver attestation form, and I believe a physicians "statement" was also sent as well. Most places give a checklist and all of the required paperwork. So long story short.... can somebody give me a correct checklist of all of the required documents that need to be sent to the state?

if already registered with the state a "change form" pack will be needed. That will include a caregiver attestation along with the change form itself, and a ten dollar fee to the state. If not a patient already an application will need to be sent along with it, and the appropriate fees are included in the directions. hope helps

the check list is listed there at the link. change form is what you need, follow the directions. your new patient is not registered yet so you will have to follow both directions, for registration, and change form , caregiver attestation, drivers licenses, doctor paperwork, and fees
