Shipping Question

I was approved for a card and filled out all the required forms. For my mailing address on the form, I have my home address down. However, due to who I am living with currently, I would not like the card to be sent there. Can I use a different return address or do anything to get the hard copy of the card shipped elsewhere? Alternatively, does the letter have anything on the outside that would give away what is inside?

You can use a post office box as your mailing address on the form.

The Letter they sent with your card, is for sure a telltale sign, Only a dummie could'nt figure out where it came from. my 2 cents.

I filled out the form and put down my home address (the one I don't want the card to be shipped to anymore) when I first went to the certification center to get a physician's signature. If I write a different return address on the envelope when I ship the money order, will it be shipped there instead?

couple things:

 

to change your addy, call LARA or change it on the application form.

about return addy? they probably ignore it.

 

about anything on the letter, small letters that say "MMP" on the envelope and its from licensing regulatory agency
