Dropping A Patient (Forms)

I of all people should know this but I never had it happen till now. So here is the question.

I need to drop a patient. The patient is not able or wanting to sign the change form. They were signed before I started having patients sign a blank change form. I reviewed the forms on MDCH site and they are all set up to have a patient sign them for a change in their CG's.

I have called MDCH and emailed with no reply as of yet.

Has anyone went down this road and what was the fix?

Can I send in a un-signed change form with a letter explaining this patient is being dropped by me? This cant be the first time this has come up.

Another thread on here suggested registered letters sent both to the patient and MDCH. As you say, though, there doesn't seem to be an appropriate form for this.

Yes. I contacted MDCH about this a while back, and this is exactly what I was told to do.
