Caregivers On Cards

I just saw my first card and it had a patients former caregivers name, adress, etc. Are we allowed to use a P.O Box for our adress, it doesn't seem to safe to have that info listed. Any help would be appreciated.

I did not mean to mis lead, I meant as a caregiver. Not as a patient. Does that make any diference? I surely do not want my home address on the back of a patients card. Thanks.
