Dear Brothers and Sisters,

I have had the distinct priveledge of being a Caregiver for a young man in my community who went to High School with my daughter. He has had Cancer for five years. Last week he went home to be with the Lord. He was 19 years, 9 months and 13 days old. Tell me that isn't bad to die that young. The kid never whined or complained. I think he always new. So if you feel inclined, say a prayer for him. My question is does anyone know what paperwork I need to submit to the State to close him out??? I appreciate it.

My condolences brother. We lost a patient to cancer not too long ago. Great gal in her 30's. Oh so too young.

 

You can get in touch with the MDCH and let them know but really all you need to do is destroy the card. Or send the card to the MDCH with a letter letting them know.

Sorry for the loss. At least now his pain is gone for good.
