Switching Caregivers Immediately

I just renewd my card with a caregiver for the first time but a carigver who is my friend wants me as a patient now. what do i have to do to switch, i havent recived my card card yet but my papers are legal.

you'll need to complete and  file a change form and submit a new completed caregiver attestation form   http://www.michigan.gov/lara/0,4601,7-154-35299_63294_63303_51869---,00.html

 

They'd like you to wait some time for changes to your registry, they're rules are at the link

Send in the change form via registered mail with a return card 28 business days from day of the change forms arrival your good to go.

Recommend doesn't stop it from going through it just slows the process have  done this multiple times over the last 4 years as long as the change form is dated after your renewal it should be no issue..

same here, never had a hiccup making changes,  but I feel I should start with the facts with a newbie in case he didn't see the rec at the LARA website.

60 days in no issue. ive had my card for at 5 years now. ive just always been my own caregiver.
