t-pain Posted February 26, 2015 Report Share Posted February 26, 2015 (edited) lara denies for the following reasons:money order or check problemform not filled out correctly, includes drivers license name/addy not matching form name/addyused old form, old forms are rejected. please use new forms only - the forms are updated like twice a year for no reason. missing copy of drivers license or bad copy that they cant read. theres a few more reasons i'm not remembering right now. maybe forgot to sign a form or put a date on there. It's probably the card but then again it could be a denial letter. Hoping it's the former. Now I just need to work on finding a caregiver as there are no dispensaries in my area (Battle Creek). Thank you. probably filled the forms in wrong (BTW they are not "...denied for MMJ...", but the forms from the State were sent back and they usually note what the issue is in the cover letter.. Once again thank you all. I received a certified letter from LARA when I was rejected initially. The Dr. had transposed my birth date on her recommendation form. You will likely need to return to your doctor regardless of what the letter says because they won't accept any copies so more than likely will need an entirely new recommendation. Best of luck.
