Denial letter. What now?

So I got a denial letter because they didn't like my signature date(my fault 1 and 8 too close together). They said to send in a new application along with my license copy, and most recent physician referral. 

 

My questions are 

1. Do I need to go back to where i got my certification and get another one written up? Or is a copy of rhe one I have okay?

2. Am I able to print off the application somewhere online?

3. How long will it take to hear back from the state this time? Also should I send it certified?

yes, the physician will give you another form usually no problem.

you can print the forms off at lara website
http://michigan.gov/mmp

certified gives you better feeling no doubt. otherwise lara drone on phone says "we didn't get it".

Do I have to go back and get a new one if I still have a copy of the last one(got the last one April 26th 2018)

Send the new information that is requested soon. It will take another month to get the card though.

A friend just went thru this same thing. Finally got card a month later.

Believe they ask for a copy of letter they sent you of rejection.
