LaceyV Posted June 2, 2018 Report Share Posted June 2, 2018 So I got a denial letter because they didn't like my signature date(my fault 1 and 8 too close together). They said to send in a new application along with my license copy, and most recent physician referral. My questions are 1. Do I need to go back to where i got my certification and get another one written up? Or is a copy of rhe one I have okay? 2. Am I able to print off the application somewhere online? 3. bax Posted June 2, 2018 Report Share Posted June 2, 2018 (edited) yes, the physician will give you another form usually no problem. you can print the forms off at lara website http://michigan.gov/mmp certified gives you better feeling no doubt. otherwise lara drone on phone says "we didn't get it". A friend just went thru this same thing. Finally got card a month later. Believe they ask for a copy of letter they sent you of rejection. Good luck.
