Renewal With A Caregiver?

Do i need a copy of a photo ID, of my caregiver?? Filling out form right, thanks for a response.

I am not so sure?? i have the  same Caregiver, and they/me sent a photo ID copy then. I don't see on the revised form, where i need to resend my Caregivers photo ID. Any body else got an answer. i need to renew now, my Caregiver is gone for a week. i confused what to do, i have my Doc,s Recert...

it says on the first page of the form, include a copy of both ID.

Not seeing the word, "BOTH"

Not seeing "BOTH" on the Form that was Revised on 1-15.

 

(I know that two years ago, we needed to send both.)

The form doesn't actually say both, but if you check both boxes for patient and caregiver, I'm thinking you need photo ID for both.

Just got off the phone with Lara.

 

The Caregiver I.D. is NOT "required" for a Caregiver that is currently still Active with other Patients.

 

It is NOT "required" for new Caregiver, who has never been a Caregiver, however, it is "recommended" to send it in.

 

Makes sense to me.
